The Hospital Licensed Social Worker (LSW) provides direct service to patients and their families to promote optimal psychosocial functioning, increase coping with illness, and assistance with appropriate post hospitalization care, based on age and disability. In collaboration with the Case Managers, LSWs assist patients along their continuum of care.  LSWs provide the highest quality of care at a reasonable cost, in accordance with the rules and regulations of federal, state, agency and the National Association of Social Work Code of Ethics. LSWs demonstrate the philosophy, mission, and values of the hospital during all interactions with patients, families, physicians, staff, and other customers with the intent of furnishing the highest satisfaction to these individuals.

Along with the Case Manager, meets with patient and family to provide counseling and works collaboratively with the patients, family and health care team to develop a discharge plan that meets the needs of the patient.  Arranges for DME, transportation, community referrals, skilled or intermediate nursing home placement, rehab, foster care, and other discharge needs as required.

Works with community agencies to assist patients in getting all discharge needs met.

Works with insurance companies to get authorizations and precertification for post-acute services.

Assesses the patient’s ability to purchase medications, supplies and DME.  Provides resources when they are available.

Assess the psychosocial needs of the patient and discuss needs with hospital interdisciplinary teams as needed.

Meet with patients and their families to discuss barriers to discharge.

Refer to outside agencies as necessary and required by law for the safety of the patient population.

Maintain required continuing education to maintain competency in their area of work.

Responds to the Emergency Department when requested to assist with psychosocial problems and referrals to external agencies.

Act as a patient advocate to ensure the patient’s safety, rights, and dignity of life are maintained. Reports to the appropriate regulatory agency all suspected child and elder abuse.

Assesses OB patients for the following; teenage mothers or mothers suspected or known to have an alcohol or chemical dependency, adoption issues, bonding issues, children’s services referrals or other referrals as needed.

Along with the Case Manager, initiates timely discharge planning and reassesses and revises the plan as required.  Arranges for the placement of patients at an appropriate level of care in compliance to the requirements of third party payors and federal and state regulations.  Promotes patient and family choice for appropriate services such as intermediate care, skilled nursing, rehabilitation, home health care, hospice, foster care, group home, drug and alcohol treatment, or other specialized programs.

Minimum Qualifications

Minimum Years and Type of Experience:

One or two years’ experience in an acute care hospital, working with medical/surgical, OB and pediatric patients.  Knowledge of age specific competencies and internal and external resources available to these patients

Other Knowledge, Skills and Abilities Required:

Ability to multitask and work in an ever-changing environment. Interpersonal skills necessary to interact effectively with staff physicians, insurance companies, department associates, and outside agency personnel to meet the discharge needs of the patients.
