RN-Clinical Quality Lead- Hampton Roads Medical Groups

The RN Clinical Quality Lead will be a clinical leader supporting the staff and providers in the medical group practice setting with a focus on improving quality of patient care and achieving key performance indicators. This role provides guidance and support to the team members, implementing continuous quality improvement efforts and accountable for maintaining progress to success. Responsibilities include education and training on standardization of workflows and best practice for achieving patient care quality metrics. Appropriately communicates the organization’s vision, values, key drivers and operational needs to the team.

Essential Job Functions

  • Encourages and works closely with providers and care teams to implement process and quality improvement activities and workflows necessary to promote high quality and cost-efficient care. Is a professional role model and provides leadership in motivating and inspiring the team to keep projects moving toward successful completion appropriately elevating any potential issues, challenges, barriers and success stories with operations leadership.
  • Provides education and clinical training as needed for medical assistants in the practices and acts as a mentor, including providing input to operations leadership regarding performance of staff competencies and skills check lists.
  • Assists providers with monitoring their PC Dashboards and and managing gaps in patient care, including opportunities for outreach to improve Annual Wellness Visits completion.
  • Works closely with the data analytics team and EPIC EHR team to optimize access to accurate and actionable quality performance data for use by the practice to improve patient care and services, population health management, and reduce the total cost of care.
  • Facilitates daily care team huddles and pre-visit planning activities in the practice.
  • Collaborates with the population health care team to leverage success with patient health maintenance goals and quality needs.
  • Conducts nurse visits with patients, face to face and/or virtual to address preventative health topics, medication review, any quality care gaps and perform appropriate screenings.
  • Identifies opportunities to improve Risk Score Accuracy through Hierarchical Condition Categories (HCC) documentation and facilitates education for providers toward success.
  • Acts as a resource and directs activities for patient education and orientation to the practice.
  • Participates in regular practice meetings with providers and staff. Prepares and gives presentations in a professional manner.
  • Attends required training, education and learning events by webinar, teleconference, virtual or in person.

Employment Qualifications

  • Licensed to practice nursing in Virginia or legally eligible to practice in Virginia based on the Nursing Compact. 
  • Bachelor of Science degree in nursing or healthcare administration (required)
  • Masters in healthcare administration or related field (preferred)
  • Thorough knowledge of the nursing process and clinical nursing practice.
  • 5 years minimum work experience in healthcare with well-rounded clinical experiences including work in acute and/or outpatient settings.
  • Has a thorough understanding of the clinical competencies and skills requirements and regulations of the medical assistant (MA) scope of practice. Subject matter expert in medical practice quality and key performance indicators. Strong analytical skills; Ability to manage multiple simultaneous projects; Work independently and proactively in stressful medical environment; Attentive to details and accuracy; Able to manage time, priorities and resources; Strong verbal, written and interpersonal skills with the ability to adopt communication style to engage target audience; Experience and working knowledge of Microsoft office Word, Excel, and PowerPoint; Ability to travel locally and regionally; Ability to plan, hold and facilitate meetings and networking events.
  • Experience with principles in continuous quality improvement, adult learning principles, and program planning required.
  • Must have excellent communication, interpersonal relationship skills, internal consulting skills and above average computer skills.
