Job Summary

The Population Health Clinical Director’s is to provide clinical and strategic leadership to the Population Health initiatives. The position is expected to drive value based outcomes through clinical incites from data analysis, payer relationships, IT solutions, clinical outcomes and collaboration with partner leadership and operations. Population Health initiatives include but are not limited to quality improvement, risk score accuracy, government programs, KPIs and improvement in ineffective utilization.

Essential Job Functions

Leads the medical economic analysis for each ACO to drive the strategy and design and implementation of Population Health initiatives around quality improvement, risk score accuracy, government programs, KPIs and improvement in ineffective utilization.  

Communicates, collaborates and supports affiliate and system leaders regarding Population Health Programs within hospital, ancillary and medical group operations.

Identifies opportunities through data analysis for improvement in the group that will drive the success under Population Health Programs. Responsible for understanding helping assimilate and translate data into insights. Collaborates with system leadership and the analytics team to drive operationalize this strategy towards desired results. Drives the operationalization of necessary population health analytics, reports and EMR needs necessary to execute the strategy.

Responsible for Payer partner relationship and strategy.  Acts as a liaison between Payer partners and the Clinically Integrated Network (CIN).

Provides clear direction to Population Health Clinical Operations and Network leads on priorities through the development of effective value-based care teams. Ensures that all programs are patient centered and align with system values. Ensures care delivery model is appropriate to level of licensure.

Prioritizes implementation of existing and new clinical programs based on volumes, contract needs, and risk opportunity based on evaluation of current services

Works closely with employed and affiliated physicians to lead performance conversations and implement change management strategies

Works with Network Operations to develop network growth strategy and relationship to optimize site of service

Ensures program models support outcomes based on risk stratification for low risk, rising risk, palliative, etc.

Works directly with the executive team to ensure that the population health infrastructure is financially sustainable

Provides clear direction to Pop Health Clinical Operations and Network leads on priorities through the development of effective value based care teams.   Provides leadership and consultation to population health leads in the CIN, ACO and provider executive team

Develops a collaborative relationship with market and system counterparts and understands the market environment

Employment Qualifications

Required Education

MD or DO

Minimum Years and Type of Experience:

5 Years of experience with value based care. MBA preferred.

Other Knowledge, Skills and Abilities Required:

Superior communication skills – verbal and written. Ability to motivate, coordinate and direct team directly and through influence. Results oriented – driven towards outcomes, tenacity to make things happen. Comfortable and understands process, but keeps focus until the goal is achieved.
