The Population Health Coordinator is responsible for supporting care teams and higher quality patient care by pre-charting, conducting chart abstraction and providing feedback to key stakeholders and care team members.This role provides support and guidance to drive continuous quality improvement efforts at the patient and provider level and greater success in value based contracts. Vital responsibilities include the ability to successfully manage large volumes of data in an effective manner, access and update patient records, appropriately communicate with patients and manage relationships with key stakeholders.

Essential Job Functions

  • Supports and works closely with Program Managers to support quality improvement efforts for Population Health Patients that are covered through any value based contracts.
  • Communicate by performing follow ups with patients through phone calls or EMR messages to close care gaps, responds to patient messages from outbound screening messages.

  • Updates patient charts with abstracted screening and test results, tracks chart touches and identifies opportunities for improvement.

  • Appropriately communicates escalations when necessary. Conducts written, verbal, and electronic communication with providers, their care team, and leadership on patient specific opportunities.

  • Conducts Pre Charting in advance of Population Health Patients select patient visits to ensure the record is up to date, and appropriately prepared for any upcoming visits, such as adding reminders for the care team to close important gaps.

  • Electronically managing patient data, such as, but not limited to scanning and other CareEverywhere abstraction activities related to satisfying gaps in the patient record.

  • Conducts outreach to patients to communicate care gap closure opportunities.

  • Performs payer-specific patient research and documents findings in payer portals or other documentation system to support gap closure and demonstrated outcome delivery.

Employment Qualifications:

Required Minimum Education: Associates Degree Specialty/Major: MA, LPN or Healthcare related field

Licensure/Certification Preferred: Certified/ Registered Medical Assistant accredited by a recognized nationally organization.LPN/RN accredited by CCNE, ACEN, or CNEA.

Minimum Years and Type of Experience: 5 years’ minimum in a healthcare setting, preferably ambulatory practice with data management skills. 

Other Knowledge, Skills and Abilities Required:Ability to manage multiple simultaneous projects; Work independently and proactively in stressful medical environment; Attentive to details and accuracy; Able to manage time, priorities and resources; Strong verbal, written and interpersonal skills with the ability to adopt communication style to engage target audience; Experience and working knowledge of Microsoft office excel.

Other Knowledge, Skills and Abilities Preferred: Working knowledge of EHR’s, preferably EPIC;
