The Hospice RN functions as a member of the hospice interdisciplinary team and provides nursing services for hospice patients and families. The Hospice RN may provide nursing services at any location within the hospice’s service area including the home setting, facility, hospital, or inpatient unit and services provided are consistent with the hospice level of care (Routine, Respite, Continuous, or General Inpatient). The Hospice RN participates in the ongoing development of an individualized and holistic Hospice Plan of Care, and demonstrates competency and skills associated with palliative nursing including effective and compassionate communication with patients, families, caregivers, and members of the care team. The Hospice RN follows the Bon Secours Mercy Health code of conduct, organizations policies, and all applicable regulatory and accreditation requirements.
Provides skilled nursing comprehensive assessment of patient and environment of care including physical, psychosocial, and spiritual assessment. Assesses symptoms, safety risks, infections, disease progression, effectiveness of interventions, adequacy of caregiver, and any other factors that impact the
patient’s quality of life.
Provides nursing interventions consistent with the hospice Plan of Care and in accordance with all applicable policies, procedures, and relevant infection control techniques; Demonstrates competency in nursing skills consistent with palliative care.
Performs measures to effectively manage medications including administration, reconciliation, and assessment of medication outcomes at appropriate intervals; reviews medication list and consults with physician and/or pharmacist to assure accuracy and best outcomes for palliation of symptoms; handles medications according to hospice policies; provides medication instruction to patient/caregiver and assesses for medication side effects.
Provides education to patients/families/caregivers utilizing various tools including verbal and or written instruction, handouts, digital resources, and demonstration techniques. Provides clear teaching on end of life care including hospice program and benefits, disease progression, safety measures, medical procedures, medication administration and side effects, palliative interventions, and treatment options. Assesses patient/family/caregiver understanding of education provided.
Completes documentation in electronic medical record and submits within required time frame to assure patients’ records are accurate and include all necessary elements according to policy, nursing best practices, quality standards, and regulatory and accreditation requirements.
Participates in collaboration with the patient/family/caregiver, Attending physician/NP, and the Hospice Interdisciplinary Group to develop and perform ongoing evaluation of the Hospice Plan of Care in order to achieve the patient/family’s end of life goals; evaluates effectiveness of interventions and patient/family/caregiver’s adherence to the Plan of Care and revises and updates the Plan of Care as needed.
Works after hours shifts, weekends, and/or on-call as required and responds within appropriate time to calls and assignment changes.
Provides supervision and competency evaluation within scope of Registered Nursing practice and agency policies for License Practical Nurses, Aides, and peer RNs. Assures supervised disciplines are following Hospice Plan of Care, organizations policies, applicable regulations and accreditation standards.
Graduate of an accredited school of nursing. BSN Degree preferred.
Active license as Registered Nurse in state of Virginia
Certified Hospice and Palliative Nursing certification preferred
Minimum 1 year direct nursing care required
Home Care or Hospice experience preferred
Valid driver's license and an acceptable driving record required
