4 10 hours days a week
Performs assigned functions related to providing information about services to potential Bon Secours Hospice patient and admitting patients to Bon Secours Hospice service.
Assumes initial responsibility for assessing patient/family needs and consulting with attending physician and other hospice team members.
Makes the telephone contact and/or assessment visits with potential hospice patients to provide information on Bon Secours Hospice services and/or assess patient’s status
Determines patient’s appropriateness for admission consistent with Bon Secours Hospice service policies and admission criteria based on Physician’s determination eligibility.
Plans for admission of patients to Bon Secours Hospice in coordination with hospice in-take staff and Hospice Clinical Managers.
Admits patients, develop plan of care and complete related paperwork, in accordance with Bon Secours Hospice polices/procedures and federal regulation.
Completes all documentation and care collaboration that supports the patient’s level of care within the hospital and departmental electronic medical record
Obtains data on physical, social, and spiritual factors that may influence patient/family health status and incorporate that data into the plan of care
Manages patient/family care plan and accompanying services between time of admission and transition to primary case manager as necessary.
Assists with implementation and coordination of services required for discharge of patients from hospitals into Bon Secours Hospice program of care.
Instructs primary caregivers, volunteers and employed caregivers to provide care as indicated.
Informs the Hospice Clinical Manager, in-take staff and IDT of unusual or potentially problematic patient/family issues.
Reports off on admission/visits to appropriate staff via e-mail/phone.
Shares in providing 24-hour, seven-day-a-week coverage to patients/families when necessary.
Graduate of an accredited school of nursing. BSN Degree preferred.
Possession of current license to practice as a registered professional nurse in the state of Virginia.
Minimum of three years of experience as a professional nurse that includes hospice/palliative or oncology
Work requires that the incumbent maintain CPR certification.
Demonstrated experience in coordination of activities, good organizational skills and ability to work independently.
Valid driver’s license and an acceptable driving record
Demonstrated ability to relate effectively with patients, staff, physicians, community agencies, and institutions.
Demonstrated ability to communicate effectively with members of the hospice interdisciplinary team (IDT).
