Summary of Primary Function/General Purpose of Position (2-3 sentences)
The Ambulatory Care Clinical Pharmacist will optimize drug therapy outcomes by providing high quality, cost-effective health care; influencing prescribing behaviors; identifying, preventing and resolving drug-related problems; and educating health care personnel regarding drug therapy.
Essential Job Functions
- Participate in clinical programs to improve patient care, including but not limited to participating in in-person or telephonic pharmaceutical care clinics, therapeutic drug monitoring programs, and programs to support state-wide initiatives.
- Perform medication reviews and medication therapy management to ensure safe and appropriate use of medications at transitions of care and yearly as appropriate.
- Support measures and initiatives to increase patient safety regarding medication management
- Engage and promote cost-containment efforts towards both medication utilization and best use of pharmacy resources
- Apply appropriate techniques and strategies to effectively respond to provider/nurse consults and drug information requests.
- Contribute to and provide guidance in drug utilization appropriateness and prescribing practices
- Collaborate with pharmacy leadership team, CarePATH, quality, risk, care managers/care coordinators to ensure high quality patient care
- Adheres to compliance with all applicable rules, regulations and standards as promulgated by Federal, State, and accrediting agencies or regulating bodies. This includes, but is not limited to, the Ohio Department of Insurance, Ohio Department of Health, Centers for Medicare and Medicaid Services, NCQA, the Ohio State Board of Pharmacy, and other applicable regulatory agencies

Minimum Qualifications
- Minimum of one year of clinical pharmacy experience, preferably to include managed care or ambulatory care experience
Other Knowledge, Skills and Abilities Required:
- Completion of residency program preferable in ambulatory care. Exceptional communication (verbal and written skills) as well as the ability to establish and maintain working relationships with other health care professionals with minimal supervision. Must be highly motivated and have exceptional time and project management skills and ability to work with minimal supervision.
Other Knowledge, Skills and Abilities Preferred
- Additional certification including but not limited too, board certification, CDE, geriatric certification, MTM certification
