The clinical navigator coordinates initial care for patients in need of orthopaedic care by scheduling and registering patients for physician appointments.  The clinical navigator is responsible for appropriate clinical documentation and accurate assessment when scheduling office appointments within the musculoskeletal service line.  The clinical navigator is responsible for responding to the needs of the patients and connecting them to the appropriate services, including sending messages to the clinical team, transferring patients to appropriate Mercy and community resources, and coordinating with hospital ancillary scheduling. The clinical navigator may be asked to assist with additional duties to support Revenue Cycle and MHP practices operations, including, but not limited to referral scheduling and follow-up.  

50%Works collaboratively with the physicians to assure continuity in scheduling care for all musculoskeletal patient office appointments.Acts as the liason to staff, patients and families to ensure continuity of orthopaedic and physical medicine care.

10%Assists the physicians in developing and editing the decision making algorithm that will be utilized to accommodate the patients with the appropriate appointment based on diagnosis, availability, and location.

10%Leads and advocates patient/family-centered care for the Orthopaedic population.Develops, integrates, and applies a variety of strategies that are driven by the needs and strengths of the patient/family.

10%Acts as an effective liaison to physicians, staff, patients/families to ensure hospital services in accordance with the plan of care.

5%Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through the improvement and innovation.

5%Displays sensitivity, compassion, understanding and empathy when working with others.Is a team player and supports the mission and philosophy of Mercy Health.

5%Follows appropriate workflows to schedule patients utilizing the EPIC EMR system.

5%Performs other duties as assigned.

Education and licensure:

LICENSURE/CERTIFICATIONS (must be non-expired/active unless otherwise stated):

Required: Current AT, PT, or RN licensure in Ohio with sufficient clinical knowledge in Orthopaedics

Preferred:Bachelors degree preferred

MINIMUM QUALIFICATIONS

Minimum Years and Type of Experience:Minimum of 3-5 years working with/in orthopaedics is required

Other Knowledge, Skills and Abilities Required:Has in-depth knowledge in specialty area and demonstrates self-directed, project management, excellent communication and customer service skills.Make decisions with appropriate information.

Other Knowledge, Skills and Abilities Preferred:Coaches others by providing constructive feedback.Evaluates existing plans, programs and processes based on established objectives.Applies advanced knowledge and experience to develop new methods, programs and procedures.
